Can’t Sleep or Can’t Stay Awake? Understanding Sleep Disorders
Sleep is supposed to restore the body and quiet the mind. Yet for millions of people, nighttime does not bring real rest. They may struggle to fall asleep, wake repeatedly, stop breathing during sleep, move their legs uncontrollably, or feel overwhelmingly sleepy during the day.
A sleep disorder is not simply an occasional bad night. It is a recognized medical condition that fundamentally affects the quality, timing, duration, or structure of sleep. Over time, disrupted sleep can negatively influence concentration, mood, memory, work performance, relationships, and overall physical health.
The National Heart, Lung, and Blood Institute estimates that between 50 and 70 million Americans currently have a sleep disorder.
The Main Types of Sleep Disorders
Sleep disorders are not one single condition. They include several different and distinct diagnostic groups, each requiring a specific approach.
Insomnia: Insomnia involves difficulty falling asleep, staying asleep, or returning to sleep after waking in the night. A person may spend enough time in bed but still wake feeling entirely exhausted. Chronic insomnia can deeply affect memory, concentration, emotional stability, and everyday functioning. It may occur alone or alongside anxiety, depression, chronic pain, hormonal changes, or other medical conditions.
Sleep-Related Breathing Disorders: Obstructive sleep apnea is the most common condition in this group. It occurs when the upper airway repeatedly narrows or becomes physically blocked during sleep. Possible signs include:
- Loud or frequent snoring
- Gasping or choking during sleep
- Pauses in breathing noticed by a partner or another person
- Morning headaches
- Dry mouth after waking
- Persistent daytime sleepiness
Not everyone who snores has sleep apnea, but repeated gasping, breathing pauses, or severe daytime fatigue should never be ignored. Untreated sleep apnea may significantly increase the risk of cardiovascular problems and can actively interfere with attention, memory, judgment, and driving safety.
Sleep-Related Movement Disorders: Restless legs syndrome (RLS) creates an uncomfortable, often overwhelming urge to move the legs, usually occurring during periods of rest and most often in the evening. Moving may provide temporary relief, but the disruptive sensations can make falling asleep extremely difficult. Other movement-related conditions may cause repeated leg movements during sleep, disturbing the body's rest even when the person does not fully wake up.
Circadian Rhythm Sleep-Wake Disorders: The circadian rhythm is the body’s internal timing system, effectively our biological clock. It helps regulate when we feel alert and when we naturally become sleepy. In circadian rhythm disorders, a person’s biological clock does not match the schedule required by work, school, family life, or social responsibilities. Someone may be entirely unable to fall asleep until very late and then struggle intensely to wake in the morning. Factors like shift work, frequent travel across time zones, irregular schedules, nighttime light exposure, and certain medical conditions can heavily disturb this internal rhythm.
Central Disorders of Hypersomnolence: These conditions cause excessive daytime sleepiness that cannot be explained simply by staying up too late or having a poor night of rest. Narcolepsy and idiopathic hypersomnia (excessive sleepiness without a clear underlying cause) belong to this group. A person may sleep for many hours yet remain unusually sleepy, experience sudden, uncontrollable sleep episodes, or struggle to stay alert during ordinary daily activities. These disorders require a careful medical assessment because excessive sleepiness can also result from sleep apnea, medications, substance use, depression, neurological conditions, or chronic insufficient sleep.
Parasomnias: Parasomnias involve unusual experiences or physical behaviors that occur while falling asleep, sleeping, or waking up. They can include:
- Sleepwalking
- Sleep talking
- Night terrors
- Recurrent nightmares
- Sleep paralysis
- Confused behavior after waking
- Eating during sleep
- Physically acting out dreams
Some episodes are harmless and temporary. Others, however, may create a real risk of physical injury or may strongly indicate the presence of another underlying sleep or neurological condition.
Why Poor Sleep Should Be Taken Seriously
Sleep affects far more than just your morning energy levels. Inadequate or disrupted sleep can make it substantially harder to think clearly, remember crucial information, properly regulate emotions, react quickly, and make safe, logical decisions.
The consequences may gradually appear as irritability, reduced motivation, relationship tension, lower workplace productivity, decreased interest in intimacy, or a lingering feeling of never being fully present. Furthermore, poor sleep can drastically increase the risk of workplace injuries and motor vehicle crashes. According to the CDC, healthy sleep naturally supports attention, memory, mood, heart health, metabolism, and optimal everyday performance.
What Causes Sleep Disorders?
Sleep difficulties are sometimes connected to daily habits, but they should not automatically be blamed on a lack of discipline. Contributing factors may broadly include:
- An irregular sleep schedule
- Shift work or overnight employment
- Exposure to bright light and electronic devices late at night
- Excessive use of caffeine, alcohol, nicotine, or other substances
- Anxiety, depression, trauma, or severe emotional stress
- Chronic pain or persistent breathing problems
- Underlying neurological or hormonal conditions
- Medications that inadvertently affect alertness or sleep
- An uncomfortable, noisy, bright, or overly warm bedroom environment
Some disorders, including narcolepsy and certain specific parasomnias, may have complex neurological or genetic causes. In many clinical cases, several of these factors are intricately involved at the exact same time.
How Sleep Disorders Are Diagnosed
Diagnosis typically begins with a detailed discussion of symptoms, daily routines, medications, medical history, and daytime functioning. A healthcare provider may actively ask the patient to keep a sleep diary that meticulously records bedtimes, awakenings, daytime naps, caffeine use, exercise, and daily medications.
Polysomnography, commonly called an overnight sleep study, is a highly comprehensive test that records breathing, oxygen levels, heart rhythm, brain activity, and body movements. However, it is fundamentally important to note that it is not required for every single sleep problem.
A sleep study is most commonly used when sleep apnea, unusual nighttime behavior, or certain specific movement disorders are highly suspected. A formal narcolepsy evaluation may specifically require an overnight study followed closely by a daytime Multiple Sleep Latency Test (MSLT). Conversely, chronic insomnia is usually diagnosed through a careful clinical assessment rather than an overnight laboratory study.
In the United States, this evaluation may seamlessly begin with a primary care provider. Depending on the severity and nature of the symptoms, the patient may then be properly referred to a board-certified sleep medicine physician, pulmonologist, neurologist, psychiatrist, or another trained specialist.
Treatment Depends on the Cause
There is absolutely no single treatment for every sleep disorder; care must be highly individualized.
For chronic insomnia, cognitive behavioral therapy for insomnia, widely known as CBT-I, is definitively considered a first-line treatment. It is a structured, short-term psychological approach that directly addresses the specific behaviors, routines, and underlying beliefs that keep insomnia active. Simple sleep hygiene advice alone is often not enough for persistent, long-term insomnia.
While sleep medications may genuinely help some patients, they should always be selected incredibly carefully. Certain sedating medications can unintentionally worsen breathing problems, create physical or psychological dependence, or cause dangerous next-day cognitive impairment.
Obstructive sleep apnea may be effectively treated with positive airway pressure (PAP) therapy, specialized oral appliances, targeted weight-management strategies, positional therapy, or, in selected clinical cases, surgery.
Circadian rhythm disorders may predictably improve through carefully timed daily light exposure, strategic schedule changes, and occasionally the use of melatonin under direct professional guidance.
Treatment for restless legs syndrome may specifically include checking bodily iron levels, directly correcting any iron deficiency, carefully reviewing current medications that could worsen symptoms, and using an appropriately selected prescription treatment. Current American guidelines recommend a highly individualized approach rather than automatically relying on older, traditional medication strategies.
Narcolepsy and idiopathic hypersomnia are most commonly managed by a dedicated sleep specialist using a combination of wake-promoting medication, scheduled daytime sleep strategies, and highly practical safety planning.
Parasomnia treatment may primarily focus on improving environmental safety, actively reducing sleep deprivation, identifying and removing behavioral triggers, and properly treating any underlying sleep, medical, or neurological disorder.
Supporting Healthier Sleep
Healthy daily habits cannot cure every severe sleep disorder, but they can undeniably strengthen medical treatment and proactively protect the body’s natural biological rhythm.
Helpful, actionable steps include maintaining a strictly regular bedtime and wake time, keeping the bedroom environment consistently dark and cool, actively limiting caffeine consumption later in the day, avoiding heavy evening meals and alcohol near bedtime, exercising regularly, and firmly turning off all electronic devices well before sleep.
Most adults biologically need at least seven hours of sleep nightly, although individual physiological needs can certainly vary. More importantly than just hours logged, sleep should genuinely feel restorative and refreshing.
Persistent daytime exhaustion is not something a person should ever simply learn to tolerate. Loud snoring accompanied by breathing pauses, dangerous levels of daytime sleepiness, repeated unusual nighttime behaviors, or insomnia that relentlessly continues for months thoroughly deserves professional medical attention.
Healthy sleep is never wasted time. It is fundamentally one of the absolute core foundations on which physical health, emotional stability, clear cognitive thinking, and daily personal safety entirely depend.
